Private Duty Nursing Services Requirements

To initiate Private Duty Nursing (PDN) Services for a student on a Comal ISD campus, the Admission, Review, and Dismissal (ARD) Committee must determine that PDN services are necessary for the student to access their educational program.
 
In addition, Comal ISD Health Services must receive the required documentation from the 
contracting agency or independent contractor within the designated timelines.

Required Before Services May Begin

The following items must be completed and submitted prior to the nurse providing services 
on campus:

Parent/Guardian

  • Review and sign the Agreement for Private Nursing Services.

Private Duty Nurse

  • Review and sign the Agreement to Provide Nursing Services.
  • Complete the Background Check/FastPass Fingerprinting process.
  • Review and sign the Private Duty Nurse Handbook.
  • Upload a copy of current nursing licensure.
  • Upload current CPR certification.
  • Upload the student's current Plan of Care.

Annual and Provider Change Requirement

The documents listed above must be submitted:
  • Prior to the start of each school year
  • When there is a CPR or license renewal
  • Updated plan of care once expired
  • Whenever there is a change in the nursing provider, or
  • Before services begin for a newly enrolled student requiring Private Duty Nursing 
    services.
Please note: All required documentation must be received, reviewed, and approved by Comal ISD Health Services and Human Resources before a Private Duty Nurse may begin providing services on campus.
