Parent Consent Forms
New Parent Consent Forms for 2026-2027
Before the first day of the school year, districts must inform parents of each health-related service offered at the campus their child attends. Health-related services will vary by campus and district but might include services such as school counseling services, physical health screenings, first aid, management of chronic illnesses, medication administration, social skills training, substance abuse prevention, and stress management.
When notice is provided to a parent, a statement must also be provided of the parent’s right to decline or deny consent for any health-related service. Parents retain their right to make medical and health care decisions for their child and consent will be sought prior to health-care services being provided unless it is for life saving care or otherwise prohibited or not required under other existing laws.
By definition, (TEC), 26.0083 distinguishes the difference between health-care services and health-related services.
- Health-care services mean services that would meet the definition of either psychological or psychiatric examination or test or psychological or psychiatric treatment, as well as services that involve medical treatment, medical procedures or dispensing medication.
- Health-related services are short-term, noninvasive activities designed to promote a student’s mental, emotional, or physical health or well-being, and to promote healthy student behaviors.
Comal ISD offers a variety of health-related services, and this notice is intended to inform you of the services we offer. Parents/guardians have the right to make decisions regarding their children, and may decline or opt out of specific health-related services.
