Athletic Forms, Now Available through Rank One
All Comal ISD athletes entering the 7th through 12th grade must have an ANNUAL physical examination. The physical must be dated after May 1, 2023, to be accepted for the 2023-2024 school year. All physicals will expire at the end of each school year no matter the date of the exam.

- Follow directions to set up Parent Account. This must be done in order to download the Medical History form after completion.
If you have not completed new student registration or are unable to login to Rank One to create a profile (you must have a Student ID#) the required forms are temporarily available in the link below. Please complete and return to your Athletic Trainer or Athletic Coordinator.
