1. Prescription and non-prescription medication to be given longer than 10 days will be administered when it is accompanied by written instructions from a physician and a signed parent consent form.
2. Prescription and non-prescription medications for short term use, 10 days or less, will be administered with with a signed parental authorization form.
4. All medications must be brought into the school by an adult in its original container or prescription bottle. For prescription medications, please request your pharmacist to separate the prescription into 2 bottles, one for home and one for school. Over the counter medications must be brought to school in its original container which lists the recommended dosage.
5. Students are not allowed to have prescription or non-prescription medications in his/her possession on school grounds without proper authorization.
6. Students with fever of 100.4 or higher will be sent home from school. Your child is required to be fever free for 24 hrs without the use of fever reducing medications before returning to school. If your child feels ill, has fever, diarrhea, or is vomitting , please keep them home from school to prevent the spread of illnesses.
7. I MUST have parent/guardian's current phone numbers and an emergency contact number in case your child becomes injured or ill at school. Please contact the nurse's office with phone number changes.
